Dedicated Network of Thought Leaders
The CKD Leaders Network is a multidisciplinary network of thought leaders who have come together with the common mission to define and spread a best practice model for population health-driven chronic kidney disease (CKD) management. Our network of leaders has identified key needs relating to early identification and diagnosis, patient and provider education, care transitions and referrals, and ongoing management of CKD patients, including those with comorbidities. To address these challenges, we seek to:
Provide a networking forum for CKD care experts, enabling peer-to-peer learning
Identify opportunities for CKD experts to work together with aligned industry partners to improve CKD care delivery and patient outcomes
Develop and disseminate best practices through publications and white papers
Design and implement real world evidence (RWE) and/or pilot programs
